HEALTH CARE PROVIDER DUTIES:

Our team members are asked to lead by example, maintaining a positive attitude, and working to enhance the health and happiness of the residents we serve.

Actively contribute to creating and maintaining a clean, orderly, bright, welcoming and safe living environment.

Communicate in a positive, courteous and professional manner with members and their families, co-workers, referral agencies, and the community at large.

Participate in staff meetings and in-service educational programs as needed; actively collaborate with cross-functional team, providing feedback and recommendations for enhancing the member experience and achieving overall goals and objectives.

Comply with company policies and procedures, safety and regulatory laws and standards.

Help to maintain a clean, orderly, uplifting and welcoming environment
-
Monitors patient condition by observing and writing care notes to document physical and mental condition, intake and output, and exercise.
-
Supports patients by providing housekeeping and laundry services; shopping for food and other household requirements; preparing and serving meals and snacks;
-
Assists patients by providing personal services, such as, bathing, dressing, and grooming.
-
Helps patients care for themselves by assisting with use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene.

Helps resident with mobility by assisting them in the appropriate manner to lift, turn, and re-position themselves; prepare healthy nutritional meals, clean the facility and assist with general housekeeping.

Maintains a safe, secure, and healthy patient environment by following standards and procedures; maintaining security precautions; following prescribed dietary requirements and nutrition standards;

Updates job knowledge by participating in educational opportunities.

Remains compliant with professional standards, home care policies and procedures, federal, state, and local requirements that govern the facility.

Enhances service reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to residents and the facility.

Coordinate activities with residents to ensure that they are engaged, stimulated and remain mobile and in touch with daily life, family and friends.
-
Med Tech Certified Staff – administer physician prescribed medications, logging and monitoring any effects of such medications, as needed.
JOB QUALIFICATIONS:

Great people skills and a love for seniors (A Must)
-
Ability to communicate orally and in writing (A Must)
-
Able to work independently with little supervision and in a team environment
-
Provide proof of a High School Diploma
-
CPR First Aid certification
-
Must pass a background check
-
20 Hours of In-Service Training (required by COMAR 10)
-
Provide proof of US citizenship
-
Clear TB Exam
-
Clear medical examination and medical approval to provide care services
